CFD Therapist / multiple locations available

Full-time position (40 hours weekly)

Training and Certification provided by Polara Health. Positions available in Verde Valley, Paulden, Prescott and Prescott Valley.

Cognitive Function Development therapists work 1:1 or as group facilitators utilizing a unique therapeutic modality synthesized from a wide scope of peer-reviewed clinical and academic research. Through this modality, therapists drive targeted neuroplastic remodeling in clients across a range of ages and diagnostic needs. Therapists holding advanced credentialing from the CFD Institute will also work to develop and improve less experienced CFD Therapists and will contribute to the advancement of the CFD therapeutic model


  • Participates in required CFDI training toward credentialing an Associate Cognitive Function Development Therapist.
  • Manages assigned client scheduling and attendance to meet departmental service delivery goals.
  • Provides individual and group therapy services as allowed by credentialing and certification within the therapeutic model.
  • Participates in A/CFT meetings on behalf of clients, offering unique CFDT perspective and expertise as needed.
  • Engages in other case management activities as necessary for coordination of client care, communication with clinical and responsible parties, or other activities as needed for follow-up or proper referral on associated services.
  • Ensures timely entry and maintenance of clinical records per departmental and agency policy and procedure.
  • Works internally with clinical and medical teams ensuring coordination of care and comprehensive service and treatment of client concerns.
  • Participates as a team member and attends staffing and supervision meetings as required.
  • Attends team meetings, case reviews, staff meetings, supervisory sessions, and in-service trainings as scheduled.
  • Assists in department strategic planning and implementation through participation in agency staffings, staff meetings, trainings, and other meetings as assigned.
  • Participates in training, workshops, seminars, and other continuing ed opportunities as required by agency / departmental policy and certification.
  • Performs other duties as assigned.

Education Level:  Bachelors degree 

Discipline/Field/Course of Study:

Licensed in one of the following:  No licensure is required, though preference will given to candidates holding a current AZ Board of Behavioral Health license.All CFD therapists shall hold current credentials in good standing from the Cognitive Function Development Institute. Minimum certification requirement ? Associate Cognitive Function Development Therapist.


Field:  No experience is necessary, though preference shall be given to candidates with 2 years of experience working 1:1 with individuals.

Number of Years:  2 years of experience working 1:1 with individuals is preferred.

Special Skills/Knowledge Preferred: 

*  Excellent interpersonal skills and empathy are required. Candidates must be able to provide positive engagement to broad spectrum of individuals in either 1:1 or group settings.

*  Successful candidates will be fun-loving, disciplined, and compassionate. They will demonstrate the ability to observe client behavior and communicate understanding of that behavior from the perspective of the cognitive function development therapeutic model.

*  Successful candidates will be able to sit and engage in interactive client activities for 4-8 hours at a time.

*  Successful candidates may work toward certification upon hire. Certification requirements are available upon request.

This position is considered Safety-Sensitive by Polara health.
