The Quality Management Analyst will work under the supervision of the Chief Quality Officer to ensure Polara Health provides high quality integrated care services in a professional manner which aligns with all federal, state and local regulatory guidelines. The QM Analyst plays a role in meeting annual quality management improvement plan goals. The QM Analyst will act as a source of validity, legality and goal achievement with direct care staff as the Outpatient, Residential and Primary Care Complaint Liaison. The QM Analyst is responsible for the final review and submission of all Critical Incident Reports to external parties as required.  The QM Analyst is the liaison for the grantors and aide to the CQO regarding all Quality of Care concerns.


Qualifications (Education, Credentials, Experience and Age- and/or Disability-Specific Populations)

  • Minimum of Bachelor's degree (Master's degree preferred), or 2 years of professional experience. 

  • If Licensed, maintains professional requirements and willing to provide/train for  Supervision as needed.

  • Proficient with data management, analysis and utilization of MS Office Excel. 

  • Excellent analytical and problem-solving skills.

  • Excellent communication skills, both verbal and written. 

  • Strong people skills to engage staff as department liaison. 

  • Ability to navigate electronic medical record and other databases.

  • Self-starter who is motivated, energetic, and enthusiastic.

ESSENTIAL DUTIES/RESPONSIBILITIES:

  • Displaying adherence to Polara Health's mission and vision as well as departmental goals for continuous quality improvement.

  • Supports all organizational, state, and federal policies and regulatory guidelines.

  • Collaborating with internal and external partner supervisors and staff for improved communication and efficiency in work flow.

  • Ability to take direction and execute, maintain and track assigned projects, audits and other assigned duties, independently.

  • Complete reports regarding complaints for data analysis and improvement planning.

  • Perform comprehensive chart analysis in order to provide insight on gaps in services and ideas regarding performance improvement activities.

  • Create reports and analysis of assigned projects.

  • Maintains confidentiality and excellent communication skills with partner agencies, clients and staff to provide conflict resolution and attain excellent customer service.

  • Investigate and document resolution with respect for the rights and wishes of clients, patients, visitors and Polara Health employees.

  • Maintains all reporting responsibilities regarding Critical Incident Reports.

  • Collaborate with staff and participate in all assigned committee meetings to aid in process improvement and brainstorming.

  • Preform other duties as assigned.
