Position Summary (Purpose and Scope)

Under the direction of the program manager and clinical supervisor manager of social service, the clinical social worker provides direct service to clients and their families. This includes counseling and seeking resources to answer unmet needs. The clinical social worker also provides consultation services and in-service education programs related to the psycho-social aspects of health care to other professional staff members on the healthcare team.

Essential Duties / Responsibilities:

  • Provide direct contact with the patient and/or significant others to assist in identifying problems/needs/goals, to increase the data base, and to offer support.
  • Coordinate discharge planning for Windhaven Psychiatric Hospital & CSU patients in coordination with medical staff.
  • Participate in the review of interdisciplinary individual treatment plans with patients, nursing staff and/or case management.
  • Provide documentation in the medical record to show data collected, discharge planning, and referrals given.
  • Coordinate case management and discharge planning of referrals from the Verde Valley Guidance Clinic.
  • Participate in clinical staffing, unit and residential meetings, providing relevant social work information to the team.
  • Provide support and liaison with patient family members.
  • Maintain an accurate, updated community resource list.
  • Other duties as assigned.

Qualifications

Minimum Education:  Master's Degree

Minimum work Experience:2-3 years Clinical experience

Required Licenses/Certifications: AZ Board of Behavioral Health license (preferred)

EOE/Non-smoking facility. Pre-employment drug screening/background check required. ** All candidates must be able to obtain a DPS Fingerprint Clearance card.
